We recommend completing the New Client Form before your first appointment so check-in is quick and our team has your pet’s information ready.
How do I submit my completed form?
You can email it to us, text it through our Vello interface at 865-730-8645, or bring it with you to your appointment.
Do I need a separate form for each pet?
Yes. Please complete one form for each pet so we can keep accurate records for every patient.
What if I can’t open the PDF form?
You may need Adobe Reader, which you can download for free. You can also call (615) 241-6300 and we’ll help you get the form.
Can I fill out the form at the hospital instead?
Yes. If you aren’t able to complete the form ahead of time, just arrive a few minutes early and we’ll provide one at check-in.
